The algorithm said no. That's the answer an increasing number of Americans are getting when they file health insurance claims—not from a doctor or even a claims adjuster, but from an AI system trained to maximize denials and minimize payouts.
Insurance companies are deploying machine learning models to review medical claims at scale, making split-second decisions about whether your surgery, medication, or treatment gets covered. The technology is impressive. The question is whether anyone needs it—or more accurately, whether anyone besides insurance company shareholders benefits from it.
Here's how it works: AI systems analyze claims against vast databases of medical codes, treatment protocols, and—here's the concerning part—patterns of what other insurers have successfully denied. The models can process thousands of claims per hour, flagging anything that deviates from expected patterns or costs more than algorithmic averages.
The efficiency gains are real. Cigna reportedly processes claims 20 times faster using AI than human reviewers. But speed isn't the same as accuracy, and in healthcare, getting it wrong isn't just annoying—it's potentially deadly.
Patient advocates are sounding alarms. When AI denies a claim, the appeals process often requires navigating automated systems that assume the AI was right. Overturning a denial can take months, and many patients simply give up. That's not a bug in the system—it's arguably the feature insurance companies are paying for.
The technology raises fundamental questions about accountability. When a human claims adjuster denies coverage, they can be questioned, their reasoning can be reviewed, and patterns of abuse can be identified.
